If you require any vaccinations relating to foreign travel you need to complete a travel vaccination form for each member of the family travelling and hand them into reception. The practice nurse can look into your records and place of travel to see if you require any vaccinations prior to travel.
This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to put in your travel forms as early as possible - at least 6 weeks before you travel - as you may need several appointments to actually receive the complete vaccination/s. Some of these vaccines have to be ordered as they are not a stock vaccine. If you require multiple appointments, the last one needs to be at least 2 weeks before you travel to allow the vaccines to work (the nurse will discuss this with you if necessary).
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
